NYCTCM Application for Admission
NYCTCM's admission application and other forms are available to print, fill out and send to NYCTCM. You may be able to type on the forms using Adobe Reader's typewriter tool.
Meningitis form A student is not required to have the Meningococcal Meningitis vaccine, but the student must fill out and sign the meningitis form and read the meningitis information document.

Application fee ($70) may be paid with Paypal.
Application for admission includes:
- Completed & signed application with two passport sized photos
- Three professional or personal (not a family member) recommendations. A reference form can be requested or downloaded from our website. A reference letter must include the address and phone number of the person providing you with the letter. The referring person should send the signed reference directly to NYCTCM.
- Official transcripts from each school listed on the application.
- Copies of current licenses (for health professionals)
- A statement of purpose (a short essay on the candidate’s vision of becoming a TCM practitioner and his or her reasons for applying to NYCTCM)
- A personal interview suggested, not required
- Application fee: enclose a check for the $70 application fee payable to NYCTCM or use PayPal,

- Documentation of U.S. citizenship or Permanent U.S. residency or valid visa status
- Health records: MMR immunization certificate for students born after 1957
- Transfer students: provide the NYCTCM admissions office with course descriptions to aid in determining transfer credit.
Applicants who completed their postsecondary education in foreign countries must complete regular application procedures and in addition must provide the following:
- Certification of English proficiency (via TOEFL and TSE scores) sent to NYCTCM directly from ETS
Note: Educational Testing Service (ETS) has launched a new Chinese-language version of the TOEFL® Go Anywhere website to assist students in preparing for the TOEFL® test, the most widely accepted English-language assessment worldwide. The newly released TOEFL Go Anywhere website, available at www.toeflgoanywhere.org/cn, presents an informative overview of the TOEFL test, valuable test preparation tips, and user-friendly academic tools for students. - Valid visa (unless a permanent resident or citizen of the U.S.)
Evaluation and certified English translation of educational credentials by an appropriate agency such as WES, at http://www.wes.org.>
